Head Start 0 to 5 Volunteer Application

I understand that Fresno EOC Head Start 0 to 5 does not carry medical or auto insurance covering volunteers. I am in good physical health, and to my knowledge, have no communicable disease. I have proof of Immunizations as required by the state of California Community Care Licensing and have the results of a test for tuberculosis performed not more than one year prior to, or seven days after initial presence in the center. I understand that as a volunteer I will be required to complete orientation”. I certify, by checking the agree box, that all the information above is true and correct.
