Fresno EOC Health Services participates in CalAIM (California Advancing and Innovating Medi-Cal), a statewide initiative led by the California Department of Health Care Services to improve health outcomes and quality of life for Medi-Cal members through a more coordinated, person-centered system of care.
Community Supports Available
Medically Tailored Meals/Supportive Groceries
Medically Tailored Meals or Groceries delivers nutrition-sensitive, condition-specific meals to approved Medi-Cal members to support recovery, manage chronic illnesses, and overall health.
Must meet the three eligibility criteria:
- Medi-Cal eligible
- One or more nutrition-sensitive chronic health conditions
- Referral from a licensed health care provider

Enhanced Care Management
Enhanced Care Management (ECM) is a CalAIM Medi-Cal benefit that provides intensive care coordination for members with complex health and social needs. ECM connects patients to a Lead Care Manager who supports transitions from hospitals to home and coordinates care across clinical and community systems.
Patients may qualify if they are:
- High utilizers of emergency/hospital care
- Experiencing homelessness or housing instability
- Living with a serious mental health or substance use need
- At risk for avoidable hospital use
- Need support through care transitions and social services
Community Health Worker (CHW)
Community Health Workers connect community members to essential resources, assist in navigating healthcare systems, and provide education on preventive health to promote overall well-being.

- Finding a Doctor of Clinic
- Managing your health conditions
- Connecting to food, housing, and other resources
- Help with Medi-Cal and health insurance
- Scheduling appointments
- Transportation Support
